These are footprints of a safety culture that needs to be examined. We don't know if there are other like hazards at these other facilities.

If properly structured, the panel's work will be complementary to our root-cause investigation as well as to the safety audits that BP itself is undertaking. However, the panel will function at arm's length from both BP and the CSB.

In light of recent events in Texas City, and the findings of our team, we determined that this recommendation could not wait for the completion of our full root-cause investigation next year. We also concluded that an examination of BP's corporate oversight and culture is an essential ingredient for the full understanding of the tragedy in Texas City.

Our goal is to build a culture of workplace safety in Arizona.

The bottom line is that we have serious public-safety needs on our streets that are not being met. This very modest increase in the motor-vehicle tax could help us meet those needs.

What we're looking for is to see if there are any safety issues that the FAA needs to address through some sort of action. Right now, it's too premature to speculate on any cause of action or the cause of the accidents.
