Defendants have the right to have counsel appointed for them. They don't have the right to insist on which lawyer will be appointed for them. As long as the lawyers are professional and competent, the judge is not going to let the defendant dictate to the judge which lawyer will represent him.

(Judges) will typically remove them until they agree to behave properly and have them watch proceedings on closed-circuit television.

Ultimately, the judge has to prevail. Otherwise, it's a circus. Trying to control the defendant and maintain an orderly proceeding while not violating the defendant's rights is a fine balancing act.
